Our GP's are experts at managing and co-ordinating continuity of care including hospital in/outpatient referrals, private health interventions, multiple specialist referrals, and putting together comprehensive chronic care plans to optimise your health and well-being. We take the time to listen and explain your condition, so as to empower you to maintain the best quality of life possible. One of our oldest patients is >100years old, and only 20% of his heart works, but he lives comfortably and with dignity because we are attentive to manage his multiple chronic conditions carefully.
